INTERESTING ECHO OF THE MONTH

Case History
A 59 yrs male with a history of IHD – was referred for Echo for preoperative fitness. His echo clips are shown. What is the Diagnosis??
Regional wall motion abnormality consistent with anteroseptal MI and a large LV apical clot The echo clip of apical four chamber view shows Akinesia of all the apical segments with a suggestion of a large sessile thrombus at LV apex. LV opacification with Intravenous contrast (perflutren lipid microspheres, DEFINITY, LANTHEUS Medical imaging), confirms the presence of the apical clot (arrow).
