Low Risk Patients
Amoxycillin 3.0 g PO 1 hour before and 1.5 g PO 6 hours after.
Penicillin Allergy
Erythromycin 1.0 g PO 2 hours before and 0.5 g 6 hours after
Alternative
Cilndamycin 300 mg PO 1 hour before and 150 mg PO 6 hours after
High Risk Patients (Prosthetic Valves, Patients With Prior Endocarditis)
Ampicillin 2.0 g IM or IV and
Gentamcycin 1.5 mg / kg IM or IV, both 30 minutes before; and
Amoxycillin 1.5 g PO 6 hours after.
Penicillin Allergy
Vancomycin 1.0 g IV over 1 hour starting 1 hour before
For Gastrointestinal And Genitourinary Procedure
Ampicillin 2.0 g IM or IV and
Gentamcycin 1.5 mg / kg IM or IV (Not more than 80 mg) 30 minutes before procedure and
Amoxycillin 1.5 g PO 6 hours after.

Vancomycin 1.0 g IV over 1 hour starting 1 hour before and
Gentamycin 1.5 mg /kg IM or IV (Not more than 80 mg) 1 hour before
Low risk patients with minor procedure
Amoxycillin 3.0 g PO 1 hour before and 1.5 g 6 hours after.
