Low Risk Patients
Amoxycillin 50 mg/kg orally 1 hour before then 25 mg/kg 6 hours after 1st dose.
Penicillin Allergy
Erythromycin 20 mg/kg orally 2 hrs before and 10 mg/kg 6 hours after 1st dose.
Alternative
Cilndamycin 10 mg/kg orally 1 hour before and 5 mg/kg 6 hours 1st dose.
High Risk Patients (Prosthetic Valves, Patients With Prior Endocarditis)
Ampicillin 50 mg / Kg IV/ IM and
Gentamcycin 2.0 mg/Kg IV / IM 30 minutes before procedure and
Amoxycillin 25 mg / Kg orally 6 hours after.
Penicillin Allergy
Vancomycin 20 mg / Kg IV over 1 hour starting 1 hour before
For Gastrointestinal And Genitourinary Procedure
Ampicillin 50 mg / Kg IV / IM and
Gentamcycin 2.0 mg / Kg 30 minutes before the procedure and
Amoxycillin 25 mg / Kg orally after 6 hours
Penicillin Allergy
Vancomycin 20 mg / Kg IV over 1 hour starting 1 hour before and
Gentamycin 2.0 mg/Kg IV /IM, one hour before operation.
